Cystic Fibrosis related diabetes (CFRD) is a complication of cystic fibrosis and it is not the same as diabetes mellitus type 1 or type 2. The destruction of insulin islets causes relative insulin deficiency and there is probably a role in insulin resistance as well, especially in exacerbations or chronic progression of pulmonary disease. The development of CFRD is associated with worse lung function, poorer nutritional status, and more chest infections.

It was found that symptoms are not a good method for screening for CFRD as most patients do not present with symptoms at all.
Diagnosis is based upon findings from medical history and physical examination. The patient must suffer from cystic fibrosis. Further evaluation may include blood tests for sugar levels. There is a recommendation to screen CF patients beginning by 10 years of age with an oral glucose tolerance test (OGTT).
CFRD is treated with insulin and a glycemic controlled diet.
