Severe Foot Pain - Need Advice

Not sure what this is, maybe a reoccurring injury? Severe foot pain, left foot I have had a small hairline fracture before. Right foot (& knee) i sprained and strained at the same time. Sometimes i wake up with them and cannot walk. It can go from my ankle, to the outer side foot, to the top of the foot. Been to physical therapy & it temporarily helped 80% of the time. Only thing that helps is time. Ice worsens the pain, been taking Naproxen for them and if I get lucky, I’d wake up with them better & able to walk. Once both feet got triggered at the same time & i was bedridden. (had to crawl to the toilet) Been given cortisone shots in ankle from a rheumatologist a few times (suggested from a hospital visit)
