Hormones should keep progressing you in transition (I’m 19 and have been on the same relatively high dose for a few years and am still getting changes, my friend who is 24 has been getting HRT for less time and is getting more changes). It really is up to what you want. Surgeries are a thing that you can look into or peruse. There’s no real next step, it’s about what makes you feel right and what you want out of your transition.
