I go to Pilates, I find having a scheduled session time for exercising is a lot easier and I am looking at adding other classes soon potentially. Another way is by having someone else going with you, motivating eachother to go swimming or for a walk etc. Swimming is a good option for back issues as it's using the muscles throughout your body without the pressure of gravity on you as much.

I hear setting aside specific times of the days you want to workout helps as it sets a routine. Google has been my friend when looking for specific stretches and exercises for whatever ails me at the time

I don't know any specifically, but there are a lot of good yoga guides on youtube for specific parts of the body, if that's something you'd want to look into!

To maintain a good workout schedule, you can try setting aside time for exercise right after waking up or after meals, as these are activities you do daily. For back stretches, consider the silver bullet stretch from the Egoscue method: lay on your back with your feet up on a couch or chair, forming right angles with your knees and torso. As you breathe into your stomach, your hips and lower back will slowly move to the floor, helping alleviate back pain.
