I would send her an email requesting an appointment if you need one. If she doesn’t respond and works through a practice or hospital I would contact the office and ask when she is expected to return.

Maybe reach out to the company she works for, if there is one, and ask about her? Or any family members or friends if you know any. If she's away for a while, you can try substituting with a different therapist or someone close you trust.
