Severe cramping during orgasm with nuvaring birth control

Endometriosis and POTS/potential HEDS. I have a nuvaring birth control inside all the time to avoid bleeding and cramping per my doctor’s instructions. (Tmi warning) However, sometimes when I orgasm especially if it’s by myself or with a toy, I get really bad cramping like a period usually peaks at for me hence why I avoid my periods. The other day it was so severe immediately I ran to the bathroom to try to relax and I ended up throwing up from how bad the pain was. It was a 9/10, for me my tattoos and piercings peak at a 8/10 max usually but not for sustained time, this was unbearable. But it goes away always within about 30 minutes, but for 10-20 I’m in agony, intensely dripping in sweat as well. My dr said to just take Advil for it since there’s not a lot else we can do. Does anyone else experience this or have any advice? Ig I could take Advil before doing it but I’m not sure that will help it completely.
