Requesting Work Accommodations and Medical Evidence from Doctor

i have drafted out a letter to my doctor to ask for extra support within work and for applying for PIP, anyone think i should change/add anything. thought the draft may be helpful for others in this situation as well.. Dear Dr M, and to whom else this may concern. I am writing to you to request a paper copy of any formal diagnosis or suspected diagnosis that I have as well as a breakdown of symptoms and how they affect me day to day. I am asking for this as my work have asked me to provide evidence for “light duty” as I am wishing to permanently change my department for the café to the tills due to my health. I feel that working in the café is no longer safe for me as it involves heavy lifting, spending a prolonged periods of time on my feet, working with dangerous equipment (fryers, ovens, and knives). I feel that with my fainting and feeling dizzy I am worried I may harm myself or others at work and this risk would be greatly reduced if I was moved to a less physically intensive department. Working in the café is also causing my large amounts of pain all over my body which leads to me really struggling to complete tasks on my shift; making me anxious to go into work and causing many panic attacks. Additionally, I have been recently plagued by recurring migraines which leave me in a state where I cannot work. Due to me taking repetitive time off, my employer has taken issue with me missing work as I do not have sufficient medical evidence of the migraines to provide them. If you could include this in the aforementioned letter, I would be greatly appreciated as this has negatively affected my income. Due to my health and pain, I have found myself unable to work as many shifts as I used to which has led to me being unable to provide the means for me to support myself. Considering this and the increased costs stemming from my poor mobility on bad days, I have made the decision to apply for PIP and greatly appreciate it if you could back up my claim. I want to still be able to work and complete my time at college, but I will need adaptations to support this as the limitations set by my health are forming obstacles in my ability to do this safely. I hope you will understand and help me with providing evidence to put these in place. If you need any further information or evidence please contact me. Hi there i do hope you don’t mind me saying it’s not to back up your claim it’s to support it as the pip is evidence based and scored this way as in to what assistance or support you may need and how your disability affects you on a daily basis I’m sure as in my own case your doctor doesn’t know your daily struggles and the limitations this illness can put on you. The letter will be useful but I would ask citizens advice to support you with your application for pip any support helps … good luck
