Sometimes it just comes in waves when you don't expect it some days just do not feel like it's right some days it doesn't even feel real I've had this happen multiple times and it does suck but it sometimes is a sign of needing a break

Often. Usually though there is a reason of some sorts. Sometimes it makes little to no sense. For example I've gone into a depressive episode because someone made a healthy boundary. It wasn't anyone fault. I just collapsed due to trauma.
