I've noticed that caffeine for some reason is a huge trigger for a flare up. If I even have 3 sips of a coffee then I'm almost guaranteed to have FM issues much later in the day

I think both. Stress, illness, or when the barometric pressure drops. All three trigger my fibromyalgia. There is a new sleeping pill I'm trying I've been on it for a year. It is fantastic for pain and insomnia both. It comes in different strengths. I take 100 mgs right before I get into bed because it works fast. Have everything done and take it right before you get into bed. You won't even remember falling sleep. Pass this on to everyone you can. It's a wonder drug. Quetiapine 100 mgs is what I take. You wake up in the morning relaxed and not drowsy and it has actually helped a little with the severe stiffness and pain I'd normally wake up with.

Usually its stress, sickness/illness, getting off schedule, cold weather/rain and "over doing it"
