If they say you need a pay-Rep an they give you the opportunity to appoint your own firend or family .. make sure you know them 100.000%

I just applied for SSI. So far I've learned that it's good to have a paper trail documenting your diagnosis, and have information for all healthcare providers you've worked with concerning your condition(s) ready to put in your application. The person from the SSA office also told me to include every chronic condition I have, not just those that are considered disabilities on their own.
