Seeking Information on ADHD Medication Questionnaire in the UK

Hi,I’m trying to get a prescription for adhd medication in the uk and must do a questionnaire to assess the severity of my condition,can anyone tell me what the questions are like? Thanks I’m advance.

i haven’t done this exactly but from the questionnaires i have done for similar things i assume it’ll list categories such as work/school/relationships/household tasks/relaxation etc then ask u to mark how much your adhd prevents you from doing these/makes them harder or less enjoyable from 1-5 for example? i’m assuming this isn’t a diagnostic thing but rather a measure of how much it impacts your daily life

someone else who has gone through the adhd medication from a gp process would probably be able to explain more specifically but it sounds similar to when i’ve had to fill out that^
