I’ve been with someone for quite some time that while they know I have medication for transitioning, they keep telling me they have been fine with it, but now my surgeon is telling me I can have my procedure and this person keeps telling me that they won’t be there if I make the change. I don’t care if they don’t accept me for it, but why does a person stick around if they already know what you’ve been doing?

I'm sorry that you're dealing with that; that really sucks. To some extent, not accepting you after surgery seems the same as not accepting you for transition. In both cases, someone doesn't like who you want to be or see yourself as. idk if they could be persuaded differently, if there's some creative sexual thing you could agree on later, but it feels like a ridiculous thing to leave someone over.
