Nervous About College Placement Test and Accommodations for Disabilities

I’m going to college in the fall and I have to do a placement test so they know where I’m at. I have been in and out of college for quite sometime. I’m nervous because I have a developmental learning disabilitiy maybe autism and I have anxiety especially with tests and math but the school has special accommodations for people with disabilities. I just need to transfer my records from my college to the school where I’m going but I’m nervous about change and I don’t do well with it that’s when I drop with a W after the add drop date. Does anyone else experience this or am I nuts?
