Feeling Lost and Insane

I honestly don't know how to put this all into words, I feel as though I've lost my whole identity, like I'm constantly viewing myself from a 3rd person view, I deal with horrific visual images of violence and yelling and I have this weird "tic" where I'll be just fine and then all of a sudden I get a huge adrenaline rush and my entire body tenses up and I end up having to kind of let it pass or I'm holding something and then suddenly I'm squeezing it with all my might. I'm just lost and confused and would like advice or answers to whats wrong with me, Its like I've gone mentally insane.
