How to Get Medicaid to Pay for Top Surgery?

hey, how do you get the ball rolling for medicaid to pay for top surgery? (and go through with it in general.) just talked to my therapist and he's cool with it, but doesn't know the process. i dont have extra money or a car/driver's licence. i live on my own but am still somewhat dependant on my parents (who dont really know im trans and im sure wouldnt support me). im 28. i have a mental health provider and medicaid. i have looked at the list of top surgery professionals and think san fransisco might be where i go. but i dont know how id get there or anything. and i have some social phobias and autism, so that makes it harder (but not impossible). anyone else going through or been through this process? any tips on overcoming red-tape type checklists or traveling/insurance logistics?
