I am going to an OBGYN soon, to see what my options are for symptom alleviation and for increasing chances of fertility. I have heard metformin and prolactin are quite helpful, but is there any other things I should inquire about/bring up? I am currently taking myoinsitol over the counter with mild symptom alleviation.

Ozempic injections are hard, but can be very helpful if you have insulin resistance/weight loss difficulties specifically

I am on a vegan diet and it has alleviated my symptoms quite a bit. I know not everyone is open to veganism but aside from that some dietary changes are helpful like avoiding red meat and potatoes (swapping those for sweet potatoes) these foods and more increase inflammation which in turn will make the cysts more painful, and certain meats are known to increase prolactin levels which are a pain in the butt to bring back down. I'm currently on metformin & cabergoline (for hyperprolactinemia) and that with the diet change I have much less painful periods and actually more regular but it is not a complete cure. Any little change does help though!!
